The earlier, the better! Lead times vary per project, but as a rule of thumb, please allow roughly 1 to 1.5 months per product/toy. This gives us space for creativity, we don't have to rush. Games and books take more time to make.

Standard projects include 2 rounds of revisions: 1 during the sketch phase and 1 for the final colored artwork. Additional feedback rounds can always be added upon request.
That depends entirely on what your supplier or manufacturer needs! I typically deliver high-resolution, layered PDF files. However, I can also provide native .AI (Adobe Illustrator) or .PSD (Photoshop) files. These are organized in neat groups and layers.
